Background:

  • Modified laparoscopic radical cystectomy sparing the prostatic capsule introduced in 2008.
  • Limited data exists on laparoscopic approaches compared to open procedures for prostate-sparing radical cystectomy.

Purpose of the Study:

  • To describe the modified laparoscopic radical cystectomy technique with prostate capsule sparing.
  • To present preliminary outcomes of this technique in selected bladder cancer patients.

Main Methods:

  • Prospective study of 20 patients undergoing laparoscopic radical cystectomy with prostate capsule sparing (2008-2012).
  • Patient selection based on clinical and imaging criteria.
  • Evaluation of oncological and functional outcomes.

Main Results:

  • Mean follow-up of 36 months in patients with a mean age of 58 years.
  • No bladder cancer recurrence observed; one patient with pT3 pN1 Mx died of disease progression.
  • 90% preserved sexual function and satisfactory continence achieved.

Conclusions:

  • Laparoscopic prostate-sparing radical cystectomy is a feasible alternative for selected patients.
  • This approach may offer improved functional results with low disease recurrence rates when performed in experienced centers.
