Bone Marrow Sampling and Transplants01:22

Bone Marrow Sampling and Transplants

2.3K
Bone marrow transplant is a potential cure for several diseases, including cancer and specific genetic disorders. Notably, this procedure is applicable for patients suffering from aplastic anemia, certain types of leukemia, severe combined immunodeficiency disease (SCID), Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma, thalassemia, sickle-cell disease, and certain cancers.
The transplant begins with high doses of chemotherapy and radiation treatment, which aim to destroy...

Summary

Microfilariae of Wuchereria bancrofti were incidentally found in a bone marrow aspirate of a patient presenting with backache. Screening bone marrow aspirates in filariasis-endemic areas can identify asymptomatic carriers.

Area of Science:

  • Medical Parasitology
  • Hematology
  • Public Health

Background:

  • Filariasis, caused by Wuchereria bancrofti, is a significant cause of morbidity in India, often leading to elephantiasis.
  • Diagnosis typically involves detecting microfilariae in peripheral blood, sometimes aided by Diethylcarbamazine citrate provocation.
  • Microfilariae can be incidentally found in various aspirated samples, not just blood.

Observation:

  • A 65-year-old female presented with backache and investigations revealed a leuco-erythroblastic blood picture.
  • Bone marrow aspirate showed metastatic deposits and incidentally, Wuchereria bancrofti microfilariae.
  • The patient lacked typical clinical signs of lymphatic filariasis.

Findings:

  • Microfilariae in bone marrow aspirate, without overt filariasis symptoms, is rare and often incidental.
  • Peripheral blood examination may not always detect microfilariae or show eosinophilia in such cases.
  • This case highlights the potential for asymptomatic microfilaremia detectable in bone marrow.

Implications:

  • Screening bone marrow aspirates for microfilariae in endemic regions is crucial for identifying asymptomatic carriers.
  • This approach can aid in understanding the true prevalence of filariasis.
  • Early detection of asymptomatic carriers is vital for effective filariasis control programs.
