Background:

  • Soft tissue sarcoma (STS) survivors may face cardiotoxicity after treatment.
  • Population-based data on cardiac outcomes in STS patients are limited.

Purpose of the Study:

  • To investigate the incidence of left ventricular ejection fraction (LVEF) decline, heart failure, and cardiac death in adult STS patients.
  • To identify risk factors for cardiotoxicity following STS diagnosis.

Main Methods:

  • A regional cohort (N=100) assessed LVEF decline using cardiac imaging data (1997-2011).
  • A national cohort (N=1,187) examined heart failure and cardiac death incidence from STS diagnosis until 2012.
  • Statistical analysis included incidence rates and hazard ratios for risk factors like doxorubicin and cardiovascular disease.

Main Results:

  • LVEF decline incidence was 16.8 and 108 per 1,000 person-years in regional cohorts.
  • National cohort incidence rates were 7.3 for heart failure and 2.7 for cardiac death per 1,000 person-years.
  • Doxorubicin treatment and pre-existing cardiovascular disease were key predictors of heart failure.

Conclusions:

  • Left ventricular ejection fraction decline is a significant concern in soft tissue sarcoma patients.
  • The incidence of LVEF decline exceeded that of heart failure or cardiac death in the studied Danish population.
  • Findings highlight the need for cardiac monitoring in STS survivors.
