Background:

  • Effective colorectal cancer (CRC) screening requires informed decision-making by participants.

  • Assessing decision-relevant knowledge, decisional conflict, and informed decision-making in relation to HL is crucial for improving CRC screening uptake.

Purpose of the Study:

  • To compare decision-relevant knowledge, decisional conflict, and informed decision-making about CRC screening participation between individuals with low and adequate health literacy (HL).
  • To identify disparities in understanding and decision-making processes related to CRC screening based on health literacy levels.

Main Methods:

  • A survey was conducted with 141 participants (71 low HL, 70 adequate HL) eligible for the Dutch organized CRC screening program.
  • Knowledge, attitude, intention to participate, and decisional conflict were assessed post-information review.
  • Health literacy was measured using the Short Assessment of Health Literacy in Dutch, and informed decision-making was analyzed using the multidimensional measure of informed choice.

Main Results:

  • Adequate knowledge of CRC and screening was observed in 64% of participants (low HL: 61%, adequate HL: 67%).
  • 57% of participants were informed decision-makers (low HL: 55%, adequate HL: 58%).
  • Respondents with low HL reported significantly higher decisional conflict (25.8) compared to those with adequate HL (16.1).

Conclusions:

  • Informed decision-making regarding CRC screening participation was suboptimal across both low and adequate health literacy groups.
  • Individuals with low health literacy experienced greater decisional conflict, suggesting communication barriers.
  • Further research is needed to develop and implement effective strategies for conveying decision-relevant knowledge about CRC screening to all individuals.
