Summary

Transanal endoscopic microsurgery showed similar survival to total mesorectal excision for early rectal cancer but had higher local recurrence rates. Further research is needed to clarify optimal treatment strategies.

Background:

  • Early-stage rectal cancer treatment offers good oncological outcomes.
  • Focusing on local resections is key to reducing mortality, morbidity, and improving function.

Purpose of the Study:

  • To compare outcomes of transanal endoscopic microsurgery (TEM) versus total mesorectal excision (TME) for early-stage rectal cancer (T1+T2) in Norway.

Main Methods:

  • Observational, population-based study using prospective data from the Norwegian Colorectal Cancer Registry (2000-2009).
  • Included 543 T1 and 1593 T2 rectal cancer patients treated with TEM or TME without neoadjuvant therapy.
  • Primary outcomes: 5-year relative survival and 5-year local recurrence rate.

Main Results:

  • For T1 cancer, TEM showed comparable 5-year relative survival (96.8% vs 98.2%) but higher local recurrence (14.5% vs 1.4%) compared to TME.
  • For T2 cancer, TEM had inferior 5-year relative survival (65.4% vs 93.9%) and higher local recurrence (11.4% vs 4.4%) than TME.
  • Adjusted analysis found no excess mortality for TEM.

Conclusions:

  • TEM offers comparable 5-year relative survival to TME for T1 rectal cancer.
  • TEM demonstrates inferior 5-year relative survival for T2 rectal cancer.
  • TEM is associated with significantly higher local recurrence rates for both T1 and T2 rectal cancer stages.
