Neurologic complications are common after cardiac transplant, affecting 30% of patients. Encephalopathy, often secondary to posterior reversible encephalopathy syndrome, was the most frequent complication observed in this study.
Area of Science:
- Neurology
- Cardiology
- Transplant Medicine
Background:
- Cardiac transplantation is a vital treatment for end-stage heart failure.
- Neurologic complications are frequent post-transplant, impacting patient outcomes.
- Known risk factors include medication toxicity, infections, and metabolic issues.
Purpose of the Study:
- To determine the incidence of neurologic complications in adult cardiac transplant recipients.
- To identify common neurologic issues following heart transplantation.
Main Methods:
- Retrospective analysis of medical records from 70 adult cardiac transplant patients (2004-2016).
- Data collected included demographics, neurologic symptoms, examination findings, lab results, imaging, and treatments.
- Evaluation focused on identifying and categorizing neurologic complications.
Main Results:
- The overall incidence of neurologic complications was 30%.
- Encephalopathy was the most common complication (12 patients), often secondary to other neurologic issues.
- Other observed complications included posterior reversible encephalopathy syndrome (7 patients), neuropathic pain (4), tremor (3), ischemic cerebrovascular accident (2), and drop foot (1).
Conclusions:
- Neurologic complications are a significant concern in cardiac transplant recipients.
- The study confirms a 30% incidence of neurologic complications, with encephalopathy being the most prevalent.
- Posterior reversible encephalopathy syndrome frequently precedes the development of encephalopathy.
