Summary
During surgery for infertility, a man had unusual structures found near his veins. These were identified as paradidymis, a benign finding with potential medico-legal relevance.

- These structures were noted to be similar in caliber to the surrounding varicose veins.

- Histopathological examination revealed these structures were lined by ciliated pseudo-stratified epithelium with an incomplete muscle coat.
- The lumina of these tubules were irregular and devoid of spermatozoa, consistent with paradidymis.
- The anatomical location within the spermatic cord and microscopic features confirmed the diagnosis of paradidymis.

- Paradidymis represents a benign congenital anomaly of the mesonephric duct remnant.
