The novel Customized Aortic Repair (CAR) method shows feasibility for endovascular aneurysm repair. This technique demonstrated mid-term biological safety in a sheep model, with biocompatible polymer use.

Background:

  • Endovascular aneurysm repair (EVAR) is a minimally invasive treatment for aortic aneurysms.
  • Current EVAR methods may have limitations in complex anatomies.
  • A novel approach using custom-designed devices and in situ polymerizable materials is being explored.

Purpose of the Study:

  • To evaluate the feasibility of the Customized Aortic Repair (CAR) method for endovascular aneurysm treatment.
  • To assess the mid-term biological behavior and safety of the CAR technique in an animal model.

Main Methods:

  • A novel Customized Aortic Repair (CAR) technique involving a custom balloon and in situ polymer filling was developed.
  • A sheep model with surgically created carotid artery aneurysms was used for evaluation.
  • Animals were monitored for 20 weeks using duplex sonography and histological analysis.

Main Results:

  • Successful aneurysm exclusion was achieved in 4 out of 6 animals.
  • The polymer cast demonstrated biocompatibility and non-thrombogenicity without migration or expansion.
  • No polymer emboli were detected in critical outflow tracts.

Conclusions:

  • The Customized Aortic Repair (CAR) method is feasible for minimally invasive aneurysm repair.
  • Mid-term biological safety was demonstrated in a simulated aneurysm survival model.
  • Further investigation into CAR for complex aortic aneurysms is warranted.
