Left Ventricular Mechanics in Functional Ischemic Mitral Regurgitation in Acute Inferoposterior Myocardial Infarction

Ischemic mitral regurgitation (MR) after myocardial infarction (MI) is linked to impaired left ventricular (LV) mechanics. Two-dimensional speckle tracking revealed worse radial and circumferential deformation in patients with ischemic MR.

Purpose of the Study:
- To evaluate left ventricular (LV) mechanics using speckle-tracking echocardiography in patients with acute inferoposterior MI and ischemic MR.
- To compare LV deformation parameters between patients with and without significant ischemic MR.

- Sixty-nine patients with acute inferoposterior MI were prospectively studied within 48 hours of reperfusion therapy.
- Two-dimensional transthoracic echocardiography assessed regional myocardial function and valve status.
- Patients were categorized into no significant MR (NMR) and ischemic MR (IMR) groups; 35 healthy individuals served as controls.
Main Results:
- LV ejection fraction and longitudinal strain did not differ between MI groups but were better in healthy subjects.
- Circumferential strain parameters were significantly worse in the IMR group compared to healthy subjects and the NMR group.
- Global, basal, and mid-ventricular radial strain were significantly lower in the IMR group versus both other groups.
Conclusions:
- Acute inferoposterior MI with ischemic MR is associated with impaired radial and circumferential LV mechanics.
- 2D speckle tracking echocardiography can identify these mechanical abnormalities in ischemic MR.
