Summary

Elevated lactate levels in a glioblastoma multiforme patient may indicate tumor-driven metabolic changes. This finding suggests lactate aids aggressive tumor progression and immune evasion.

Area of Science:

  • Neuro-oncology
  • Cancer Metabolism
  • Oncologic Emergencies

Background:

  • Glioblastoma multiforme (GBM) is an aggressive primary brain tumor.
  • Deep vein thrombosis (DVT) can be a complication in cancer patients.
  • Lactic acidosis is a metabolic disturbance characterized by elevated lactate levels.

Observation:

  • A 37-year-old female patient with known GBM presented with new-onset DVT.
  • Elevated anion gap and plasma lactate levels were detected.
  • Brain MRI confirmed stable, advanced GBM; other causes of lactic acidosis were excluded.

Findings:

  • The patient's elevated lactate levels were attributed to the glioblastoma multiforme.
  • Tumor-produced lactate in the microenvironment is associated with metastasis.
  • Lactate production by tumors may facilitate immune evasion and resistance to radiation therapy.

Implications:

  • Elevated lactate may serve as a biomarker for aggressive tumor behavior in GBM.
  • Targeting tumor metabolism could be a therapeutic strategy for GBM.
  • Understanding lactate's role may inform novel approaches to overcome treatment resistance in glioblastoma.
