Unicuspid aortic valve (UAV), a rare congenital heart defect, can cause severe aortic stenosis and regurgitation. This case highlights a young male with UAV, aortic stenosis, and an ascending aortic aneurysm, successfully treated with valve replacement and aneurysm repair.

Background:
- Unicuspid aortic valve (UAV) is a rare congenital anomaly often associated with aortic stenosis and/or regurgitation.
- Ascending aortic aneurysms can occur independently or in conjunction with valvular heart disease.
Observation:
- A 26-year-old male presented with syncope, revealing severe aortic stenosis and moderate aortic regurgitation on echocardiogram.
- CT scan confirmed aortic valve calcification and an ascending aortic aneurysm measuring 4.3 cm.
Findings:
- The patient underwent successful aortic valve replacement and ascending aortic aneurysm repair.
- The co-existence of UAV with an ascending aortic aneurysm in a young patient is noteworthy.
Implications:
- Syncope in young adults warrants consideration of aortic stenosis, especially in the presence of UAV.
- Comprehensive non-invasive imaging is crucial for evaluating young patients with suspected aortic valve disease and associated aortic pathology.
