The sequence of removing or doffing PPE starts with the gloves, as they are the most contaminated. Next is removal of the face shield or goggles, as they would interfere with removing other PPE. Purpose of the Study:

  • To assess the diagnostic concordance of teledermatology consultations compared to in-person evaluations.
  • To identify factors influencing the accuracy of teledermatology, including different modalities and lesion types.

Main Methods:

  • 115 patients from four Air Force stations received teledermatology consultations via virtual teleconference (VTC) and Store and Forward (S&F) methods.
  • Consultation data was compared between VTC, S&F, and face-to-face (FTF) consultations at a central facility.

Main Results:

  • Diagnosis concordance varied: VTC vs. S&F ranged from 52.5% to 75%, while VTC vs. FTF was 60.66% and S&F vs. FTF was 91.8%.
  • Accuracy was higher for localized lesions than generalized ones, particularly for papulosquamous disorders.
  • Poor camera resolution at peripheral sites was a significant factor in diagnostic discordance for VTC.

Conclusions:

  • Teledermatology is a valuable tool for patient care, especially when local specialist access is limited and travel is extensive.
  • Improving camera resolution at peripheral centers is crucial for enhancing diagnostic concordance in teledermatology.
