Background:

A 50-year-old male presented with a chronic, three-year history of asymptomatic, skin-colored papules on the penis.
The lesions gradually enlarged, causing discomfort during sexual intercourse.

Observation:

Physical examination revealed multiple penile nodules, with the largest measuring 3x1 cm.
No similar lesions were noted elsewhere on the body.
Genitourinary investigations, including tests for sexually transmitted diseases like syphilis, were negative.

Findings:

The patient's symptoms and physical findings were inconsistent with a typical fungal infection or sexually transmitted disease.
The gradual enlargement and specific morphology of the penile papules warrant further investigation.

Implications:

This case highlights the importance of considering differential diagnoses for persistent penile lesions.
Accurate diagnosis is crucial for appropriate management and patient reassurance.
Further dermatopathological evaluation may be necessary to determine the exact nature of these penile nodules.
