Area of Science:

  • Endocrine Surgery
  • Health Systems Engineering
  • Operative Efficiency

Background:

  • Developed a high efficiency endocrine operative protocol.
  • Utilized mathematical programming, process reengineering, and value-stream mapping.
  • Aimed to increase daily operations without extending operating room time at an academic center.

Purpose of the Study:

  • To evaluate the effectiveness of a novel high efficiency endocrine operative protocol.
  • To compare patient outcomes and operative efficiency between high efficiency and standard endocrine surgery days.

Main Methods:

  • A case-control study matched 72 patients undergoing endocrine operations on high efficiency days with 72 patients on standard days.
  • Demographic profiles, operative times, and perioperative complications were analyzed.
  • Turnaround times were compared between the two groups.

Main Results:

  • High efficiency operating rooms completed an average of 7 cases per 8-hour workday, compared to 5 in standard rooms.
  • Mean procedure times were similar between groups.
  • Turnaround time was significantly reduced in the high efficiency group (8.5 min vs. 15.4 min, P < .001).
  • Postoperative hypocalcemia rates were comparable (6.9% vs. 8.3%, P = .99).

Conclusions:

  • The high efficiency endocrine operative protocol achieved greater surgical throughput without compromising patient safety or procedure duration.
  • This protocol offers a viable strategy to optimize operating room utilization and reduce surgical backlogs in universal healthcare systems.
