Background:

  • Custom ocular prostheses aim to improve patient aesthetics and quality of life.
  • Objective and reproducible methods for evaluating the artistic outcome of these prostheses are lacking.

Purpose of the Study:

  • To develop a standardized and universally reproducible grading scale for the artistic outcome of custom ocular prostheses.

Main Methods:

  • A retrospective review of 50 custom ocular prosthesis photographs was conducted.
  • Two independent oculoplastic surgeons graded prosthesis characteristics (limbus, vascularity, pigmentation) on a 0-2 scale.
  • Inter-observer and intra-observer agreement were assessed using the kappa statistic.

Main Results:

  • Good inter-observer agreement was found for limbus (0.7), vascularity (0.76), and pigmentation (0.6).
  • High intra-observer agreement was achieved by both surgeons across all characteristics.
  • Kappa values indicated substantial to excellent agreement for most assessments.

Conclusions:

  • A novel grading scale for custom ocular prosthesis outcomes demonstrates good reliability.
  • This system provides an objective method for evaluating prosthetic artistry, suitable for ocularistry education and standardization.
