Maximum intensity projections (MIPs) derived from postcontrast 3D T1W gradient echo sequences significantly improve brain metastasis detection for both junior and experienced neuroradiologists. This enhanced detection was observed regardless of field strength, performing better at 3.0 T than 1.5 T.
Area of Science:
- Neuroradiology
- Medical Imaging
- Oncology
Background:
- Brain metastases detection is crucial for patient management.
- Postcontrast 3D T1W gradient echo sequences are standard for detecting brain metastases.
- Maximum intensity projections (MIPs) offer a potential method to enhance lesion visualization.
Purpose of the Study:
- To compare the sensitivity of postcontrast 3D T1W gradient echo sequences (3DT1W) versus MIP reconstructions (MIP-3DT1) for detecting brain metastases.
- To evaluate the impact of MIP reconstruction on metastasis detection rates across different experience levels and magnetic field strengths.
Main Methods:
- Prospective analysis of 95 patients with known brain metastases.
- Comparison of 1-mm postcontrast 3DT1W with 6-mm MIP-3DT1 reconstructions.
- Two independent readers (1 year and >20 years experience) analyzed anonymized, randomized studies at 1.5 T (42 patients) and 3 T (53 patients).
Main Results:
- The less experienced reader detected 542 metastases on non-MIP 3DT1W and 605 on MIP-3DT1, an increase in 36% of patients.
- The experienced reader detected 589 metastases on non-MIP 3DT1W and 621 on MIP-3DT1, an increase in 33% of patients.
- MIP-3DT1 improved detection regardless of field strength for both observers.
Conclusions:
- MIP-3DT1 reconstructions of postcontrast 3DT1W sequences enhance the detection of brain metastases.
- This improvement in detection was consistent across neuroradiologists of varying experience levels.
- The benefit of MIP-3DT1 was more pronounced at 3.0 T compared to 1.5 T.
