Background:

  • Exercise-induced vasculitis (EIV) is an underdiagnosed condition.
  • It commonly occurs after prolonged physical activity, particularly in hot environments.

Purpose of the Study:

  • To review the clinical characteristics, treatments, and outcomes of EIV.
  • To analyze patient data including 99 cases from literature and 16 treated cases.

Main Methods:

  • Literature review and retrospective analysis of patient records.
  • Histopathological examination revealing leukocytoclastic vasculitis.
  • Analysis of clinical presentation, symptoms, and treatment responses.

Main Results:

  • EIV presents as itchy, painful, or burning erythematous/purpuric plaques on lower legs.
  • Lesions resolve spontaneously within 10 days, with frequent relapses (77.5%) if triggers persist.
  • Histopathology shows leukocytoclastic vasculitis in 95% of cases; blood tests are negative.

Conclusions:

  • EIV is an isolated cutaneous vasculitis linked to venous stasis and thermoregulation issues from strenuous exercise in heat.
  • Topical corticosteroids may alleviate symptoms, and light clothing may prevent recurrence.
