Summary

Laparoscopic caudate lobectomy is a safe and feasible surgical option for selected patients with caudate lobe malignancy. This minimally invasive approach demonstrated no perioperative complications or mortality in a small patient cohort.

Background:

  • Caudate lobectomy is technically challenging due to the deep anatomical location and proximity to major blood vessels.
  • Previous technical difficulties limited the application of laparoscopic caudate lobectomy, especially for malignant conditions.

Purpose of the Study:

  • To evaluate the safety and feasibility of performing laparoscopic caudate lobectomy.
  • To assess the outcomes of this minimally invasive approach for patients with caudate lobe malignancy.

Main Methods:

  • A retrospective review of six consecutive patients who underwent laparoscopic caudate lobectomy for hepatic malignancy between September 2007 and May 2014.
  • Assessment of demographic data, intraoperative parameters, and postoperative outcomes.

Main Results:

  • All six procedures were successfully completed using a totally laparoscopic technique.
  • Mean operative time was 382 minutes, with no requirement for blood transfusion.
  • No perioperative complications or mortality were observed, with clear resection margins (R0) and a mean follow-up of 56 months.

  • Laparoscopic caudate lobectomy is a safe and feasible procedure for selected patients with malignancy in the caudate lobe.
