Self-care in COPD: an assessment project by practice and education

This was a collaborative project between education and service that focused on teaching patients with COPD and resulted in the development and testing of an evaluation and teaching tool. The process is described and analyzed using an adaptation of Lancaster's framework for successful collaboration.
