Summary

Adrenal dysfunction is uncommon in active tuberculosis patients. Standard chemotherapy, including rifampicin, does not adversely affect adrenal function, contrary to some reports.

Background:

  • Tuberculosis (TB) can cause adrenal insufficiency.
  • Limited data exists on adrenal function in active TB patients.

Purpose of the Study:

  • To assess adrenal function in active TB patients before and after treatment.
  • To evaluate the impact of antituberculous chemotherapy, including rifampicin, on adrenal function.

Main Methods:

  • Prospective study of 90 Melanesian adults with active TB (pulmonary, miliary, extrapulmonary).
  • Adrenal function assessed via basal serum cortisol and Synacthen stimulation test before and after chemotherapy.

Main Results:

  • Most patients had normal basal cortisol; some had elevated levels, none had low levels.
  • Initially, 92% showed normal cortisol response to Synacthen; 8% were subnormal.
  • After treatment, all but one patient had normal cortisol response.

Conclusions:

  • Adrenal dysfunction is infrequent in active tuberculosis.
  • Antituberculous chemotherapy, even with rifampicin, does not negatively impact adrenal function.
