Summary

Vitrectomy with platelet-rich plasma (PRP) application offers a promising rescue therapy for persistent optic disk pit (ODP) maculopathy. This treatment effectively resolved maculopathy in all patients, preventing recurrences over three years.

Background:

  • Optic disk pit (ODP) maculopathy can cause visual loss, particularly in eyes with prior vitrectomies.
  • Refractory ODP maculopathy presents a significant clinical challenge, often requiring novel therapeutic approaches.

Observation:

  • This study evaluated vitrectomy combined with platelet-rich plasma (PRP) application and gas tamponade as a rescue therapy.
  • Three patients with persistent or recurrent ODP maculopathy after previous vitrectomy were treated with PRP.

Findings:

  • Maculopathy improved within two weeks and resolved in all treated eyes between six and eight months.
  • No recurrences were observed during a three-year follow-up period.
  • Visual acuity improved in two eyes and remained stable in one eye.

Implications:

  • Vitrectomy with PRP application shows potential as an effective rescue therapy for refractory ODP maculopathy.
  • PRP may facilitate closure of the vitreous-macular/subretinal communication at the optic disk pit.
  • This approach may offer a safer alternative to existing treatments for ODP maculopathy.
