    Summary

Primary mediastinal seminoma, a rare cancer, can be effectively treated with surgery. Radiation therapy also achieves complete remission and long-term survival for unresectable cases.

    Area of Science:

    • Oncology
    • Thoracic Surgery
    • Radiation Oncology

    Background:

    • Primary mediastinal seminoma is an exceptionally rare neoplastic disease, with only 133 cases documented in medical literature.
    • This study focuses on a small cohort of patients treated at Vancouver General Hospital (VGH) between 1974 and 1986.

    Purpose of the Study:

    • To evaluate the efficacy of surgical and radiation treatment modalities for primary mediastinal seminoma.
    • To assess long-term outcomes and survival rates in patients diagnosed with this rare condition.

    Main Methods:

    • Retrospective analysis of five patients diagnosed with primary mediastinal seminoma.
    • Treatment included exploratory thoracotomy, with surgical resection when feasible.
    • Postoperative radiation therapy was administered universally; chemotherapy was used for nodal involvement.

    Main Results:

    • All five patients achieved complete resolution of the tumor.
    • All patients were alive and disease-free at an average follow-up of 42.6 months (range, 5-82 months).
    • Two out of five patients had resectable tumors; the others received non-surgical management.

    Conclusions:

    • Primary mediastinal seminoma, despite its rarity, is a treatable neoplastic disease.
    • Surgical intervention is a primary treatment option.
    • Radiation therapy is effective in inducing complete remission and ensuring long-term survival, particularly in unresectable cases.
