Background:

  • Ebola virus disease (EVD) outbreak in West Africa necessitated specialized treatment centers.
  • While tachycardia is often associated with EVD, bradycardia has been an underreported phenomenon.
  • This study aimed to document and analyze bradycardia in EVD patients and explore its underlying mechanisms.

Purpose of the Study:

  • To investigate the occurrence and characteristics of bradycardia in patients diagnosed with Ebola virus disease.
  • To analyze electrocardiogram (ECG) findings in EVD patients presenting with bradycardia.
  • To discuss potential etiological factors, particularly encephalitis, contributing to bradycardia in EVD.

Main Methods:

  • Prospective study of nine patients with confirmed Ebola virus disease admitted to a military treatment center in Conakry, Guinea, in March 2015.
  • Collection of baseline clinical data and twice-daily follow-up data.
  • Performance of laboratory analyses, including troponin testing, and electrocardiograms (ECGs).

Main Results:

  • Patients generally presented with no or moderate tachycardia (mean pulse 82 ± 27 bpm) at admission.
  • Four out of nine patients developed bradycardia (pulse < 60 bpm), including one with marked bradycardia (43 bpm).
  • ECGs revealed sinus bradycardia without conduction disturbances or repolarization anomalies; all bradycardic patients recovered.

Conclusions:

  • Contrary to expectations, EVD patients in this cohort exhibited no or only moderate tachycardia.
  • Observed bradycardia in EVD patients was characterized by sinus rhythm, without evidence of significant sinoatrial node dysfunction or atrioventricular block.
  • Encephalitis is proposed as a potential central nervous system cause for bradycardia in Ebola virus disease, in the absence of myocarditis.
