Summary

Sacral neuromodulation using InterStim is safe for patients with cardiac pacemakers, effectively treating urinary issues like incontinence and retention. This approach showed no interference with pacemakers in a small case series.

Background:

  • Increasing use of sacral neuromodulation (InterStim) for bladder dysfunction
  • Growing number of patients with cardiac comorbidities undergoing neuromodulation

Purpose of the Study:

  • To evaluate the safety and efficacy of sacral neuromodulation in patients with cardiac pacemakers
  • To describe the experience of treating refractory voiding dysfunction in this complex patient group

Main Methods:

  • Case series of 3 patients with cardiac pacemakers receiving sacral neuromodulation (InterStim)
  • Initial screening via percutaneous nerve evaluation (PNE)
  • Staged implantation of InterStim implantable pulse generator (IPG) for responsive patients

Main Results:

  • All 3 patients experienced >50% improvement in urinary symptoms
  • No observed interference between InterStim IPG and cardiac pacemakers during postoperative programming under ECG monitoring

Conclusions:

  • Sacral neuromodulation appears to be a safe treatment option for urinary dysfunction in patients with cardiac pacemakers
  • Further investigation is warranted to confirm safety and efficacy in a larger cohort
