Background:

  • Human immunodeficiency virus (HIV) infection is associated with cognitive impairments, particularly in working memory (WM).
  • Understanding the neural underpinnings and genetic factors influencing WM training effectiveness in HIV is crucial for developing targeted interventions.

Purpose of the Study:

  • To evaluate the efficacy of an adaptive working memory training (WMT) program in individuals with HIV compared to seronegative (SN) controls.
  • To investigate the neural correlates of adaptive WMT using functional magnetic resonance imaging (fMRI).
  • To examine the influence of the LMX1A-rs4657412 polymorphism on adaptive WMT outcomes in HIV participants.

Main Methods:

  • 201 participants (HIV and SN) were randomized to adaptive or nonadaptive WMT.
  • Assessments included WM tests, self-reported executive functioning, and fMRI during cognitive tasks at baseline and follow-up (1 and 6 months).
  • LMX1A-rs4657412 genotyping was performed on all participants.

  • Adaptive WMT, but not nonadaptive WMT, enhances working memory and neural efficiency in both HIV-infected individuals and seronegative controls.
  • The findings suggest that adaptive WMT could serve as an effective adjunctive therapy for cognitive deficits in people living with HIV.
  • Genetic factors, specifically the LMX1A genotype, may modulate the response to WMT in HIV participants, suggesting personalized therapeutic approaches.
