Summary

Pancoast tumors, rare lung cancers invading the chest apex, are best treated with trimodality therapy (chemoradiation then surgery) for operable cases. Inoperable tumors receive palliative radiation and supportive care.

Background:

  • Pancoast tumors are rare lung malignancies (3-5% of lung cancers) defined by invasion of chest apex structures.
  • Involvement includes ribs, nerves, sympathetic chain, and subclavian vessels.

Purpose of the Study:

  • To provide an updated review of Pancoast tumor diagnosis and treatment management.
  • To highlight current state-of-the-art treatment strategies.

Main Methods:

  • Literature search of Scopus and PubMed databases.
  • Selection of 18 relevant manuscripts for commentary based on up-to-date information.

Main Results:

  • The current standard of care for medically fit patients with resectable Pancoast tumors is trimodality therapy: chemoradiation followed by radical surgery.
  • For unresectable tumors, particularly in patients with poor performance status (PS 4) or distant metastasis, palliative radiation therapy and best supportive care are recommended.

Conclusions:

  • Trimodality therapy represents the state-of-the-art for resectable Pancoast tumors.
  • Palliative care is crucial for unresectable cases and those with advanced disease.
