Background:

  • Neuromyelitis optica spectrum disorder (NMOSD) is a severe autoimmune disease primarily affecting the optic nerves and spinal cord.
  • Ocular manifestations in NMOSD are common, but retinal vasculitis is not a widely recognized feature.

Observation:

  • A patient with NMOSD, confirmed by transverse myelitis and positive anti-aquaporin-4 antibodies, presented with neurological symptoms but no initial visual complaints.
  • Ophthalmological examination revealed active retinal vasculitis in the affected patient.

Findings:

  • This case represents the first documented instance of retinal vasculitis in a patient diagnosed with neuromyelitis optica or NMOSD.
  • Diagnostic imaging, including MRI, fluorescein angiography, and OCT, supported the findings.

Implications:

  • The presence of retinal vasculitis may indicate a broader spectrum of ocular involvement in NMOSD.
  • This finding warrants further investigation into the relationship between autoimmune neurological disorders and retinal vascular inflammation.
