Summary

Background:

  • Primary central nervous system lymphoma (PCNSL) is a rare and aggressive brain tumor.
  • Elderly patients (≥65 years) often face treatment challenges due to comorbidities and reduced tolerance.
  • Optimizing immuno-chemotherapy regimens is crucial for improving outcomes in this population.

Purpose of the Study:

  • To assess the efficacy and safety of the R-MPL and R-MP immuno-chemotherapy protocols in elderly, immunocompetent patients with newly diagnosed PCNSL.
  • To determine the complete remission (CR) rate as the primary endpoint.
  • To compare toxicity profiles between the two treatment arms.

Main Methods:

  • A multicenter, single-arm trial involving 107 elderly patients with newly diagnosed PCNSL.
  • Patients received either the R-MPL (rituximab, methotrexate, procarbazine, lomustine) or R-MP (rituximab, methotrexate, procarbazine) protocol.
  • Treatment involved three cycles followed by maintenance procarbazine, with CR assessed after three cycles.

Main Results:

  • Overall, 35.5% of patients achieved CR and 14.0% achieved partial remission.
  • The R-MP protocol demonstrated a lower CR rate (31.6%) compared to R-MPL (37.7%).
  • Progression-free survival and overall survival rates were similar between R-MP and R-MPL, while R-MP showed significantly less grade ≥3 toxicities (71.1% vs 87.0%).

Conclusions:

  • The R-MP protocol is a more feasible immuno-chemotherapy option for elderly PCNSL patients due to reduced toxicity.
  • Despite a slightly lower CR rate, R-MP offers comparable survival outcomes to R-MPL.
  • Further research and protocol optimization are warranted to enhance the efficacy of R-MP in PCNSL treatment.
