The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. Background:

  • The Zimmer Trabecular Metal Total Ankle Replacement (TM TAR) is a novel prosthetic designed for ankle arthroplasty.
  • A lateral transfibular approach has been developed for the implantation of the Zimmer TM TAR.

Purpose of the Study:

  • To detail the surgical technique for the Zimmer TM TAR using a lateral transfibular approach.
  • To evaluate the early clinical outcomes of patients who received the Zimmer TM TAR via this approach.

Main Methods:

  • A cohort of 67 patients underwent primary total ankle replacement using the Zimmer TM TAR between May 2013 and May 2015.
  • Clinical outcomes were assessed preoperatively and postoperatively at 6, 12 months, and annually using the American Orthopaedic Foot & Ankle Society (AOFAS) scores, Visual Analogue Scale (VAS) for pain, and the Short Form Health Survey (SF-12).
  • Minimum follow-up duration was 12 months.

Main Results:

  • The mean AOFAS ankle and hindfoot score significantly improved from 32.8 preoperatively to 85.0 postoperatively (p < 0.001).
  • The mean VAS pain score decreased significantly from 8.0 to 2.0 postoperatively (p < 0.001).
  • SF-12 Physical Component Summary (PCS) scores improved from 30.2 to 43.1 (p < 0.001), and Mental Component Summary (MCS) scores improved from 44.6 to 53.5 (p < 0.001).

Conclusions:

  • The surgical technique and early results for the Zimmer TM TAR via a lateral approach are encouraging.
  • This approach and prosthetic design offer a potentially beneficial treatment option for total ankle replacement.
  • Findings may supplement existing surgical guidance for healthcare providers using these implants.
