Summary

This study found no significant differences in brain volume for individuals with subacute or chronic low back pain compared to healthy controls. Larger sample sizes may be needed to detect subtle brain volume changes potentially related to low back pain.

Background:

  • Previous research on chronic low back pain and brain volume shows inconsistent findings.
  • The impact of subacute low back pain on brain volume remains under-investigated.

Purpose of the Study:

  • To investigate the relationship between subacute and chronic low back pain and brain volume differences.
  • To explore potential correlations between low back pain and specific brain regions.

Main Methods:

  • A cross-sectional study included 130 participants: 23 with subacute low back pain, 68 with chronic low back pain, and 39 healthy controls.
  • Whole and regional brain volumes were measured as the primary outcome.
  • Clinical assessments included pain duration, intensity, fear avoidance, disability, and depression.

Main Results:

  • While some regional gray matter decreases were noted in chronic low back pain patients compared to controls, no significant whole-brain volume differences were found between groups after multiple comparison corrections.
  • No correlations were observed between clinical outcome measures and brain volume.

Conclusions:

  • Subacute and chronic low back pain are not associated with significant brain volume differences when accounting for multiple comparisons.
  • Subtle changes in brain volume related to low back pain may require larger studies for detection, or low back pain may not impact brain volume.
