Summary

Chronic gastritis, a premalignant condition, significantly increases gastric cancer risk. Severe cases, especially in the antrum, pose the highest risk, particularly for intestinal-type gastric cancer.

Area of Science:

  • Gastroenterology
  • Oncology
  • Pathology

Background:

  • Chronic gastritis is a progressive inflammatory condition of the stomach lining.
  • It is recognized as a premalignant condition for gastric cancer.
  • Atrophic gastritis, a severe form, involves significant mucosal atrophy.

Purpose of the Study:

  • To evaluate the association between chronic gastritis and the risk of developing gastric cancer.
  • To identify specific risk factors and quantify the increased risk associated with gastritis severity and location.
  • To understand the subtypes of gastric cancer mediated by chronic gastritis.

Main Methods:

  • The study analyzes the relationship between chronic gastritis, its severity (atrophy grade), and location (antrum, body) and gastric cancer incidence.
  • Risk stratification is performed based on the presence and extent of chronic gastritis.
  • The study considers the specific type of gastric cancer (intestinal type).

Main Results:

  • Chronic gastritis significantly increases the risk of gastric cancer compared to normal gastric mucosa.
  • Severe chronic gastritis, particularly in the antrum (B-type), confers a 3-5 times higher risk than in the body (A-type).
  • Combined severe antral and body gastritis (AB-type) presents the highest risk. The risk is primarily associated with intestinal-type gastric cancer.

Conclusions:

  • Chronic gastritis is a confirmed risk factor for gastric cancer, especially intestinal type.
  • The severity and location of chronic gastritis are critical determinants of gastric cancer risk.
  • Individuals with severe chronic gastritis, particularly young men with antral or extensive involvement, face a substantially elevated long-term risk, potentially up to 30%.
