A rare case of patellar aneurysmal bone cyst in a 32-year-old female was treated with total patellectomy and extensor mechanism reconstruction. This surgical approach addressed significant joint surface destruction, leading to a satisfactory outcome.

Background:

  • Aneurysmal bone cysts (ABCs) are rare, benign bone tumors, typically occurring in long bones and the spine.
  • Patellar ABCs are exceptionally uncommon, presenting diagnostic and therapeutic challenges.
  • This case highlights the importance of considering rare pathologies in the differential diagnosis of knee masses.

Observation:

  • A 32-year-old female presented with a progressively enlarging, painful swelling of the left knee without a history of trauma.
  • Radiographic and MRI evaluations revealed a destructive lesion consistent with a patellar aneurysmal bone cyst.
  • The cyst had caused significant destruction of the patellar joint surface.

Findings:

  • The diagnosis of patellar aneurysmal bone cyst was confirmed through imaging.
  • Despite preserved range of motion preoperatively, the extent of joint destruction necessitated a more aggressive surgical intervention.
  • Total patellectomy and extensor mechanism reconstruction using the Zaricznyj technique were performed.

Implications:

  • This case demonstrates a successful management strategy for a rare patellar aneurysmal bone cyst with extensive joint destruction.
  • The Zaricznyj technique offers a viable option for extensor mechanism reconstruction following patellectomy in such cases.
  • Early and accurate diagnosis is crucial for optimal treatment outcomes in rare bone pathologies of the patella.
