Background:

  • Primary pancreatic lymphoma (PPL) is an exceptionally rare pancreatic neoplasm, accounting for only 0.5% of all pancreatic tumors.
  • Accurate diagnosis of PPL is critical as its management differs significantly from other pancreatic malignancies like pancreatic ductal adenocarcinoma or neuroendocrine tumors.

Observation:

  • An 81-year-old female presented with obstructive jaundice, nausea, and appetite loss, alongside elevated liver enzymes and tumor markers.
  • Imaging revealed a diffusely enlarged pancreas without a distinct mass, accompanied by bile duct dilation and ascites.
  • Endoscopic ultrasound showed a homogeneous, hypoechoic pancreas without identifiable lesions or enlarged lymph nodes.

Findings:

  • Cytological examination of fine-needle biopsy samples revealed atypical lymphoid cells with a B-cell immunophenotype.
  • Despite limitations in further characterization, a diagnosis of primary pancreatic lymphoma was established.
  • The patient's presentation mimicked acute or autoimmune pancreatitis due to the diffuse infiltrative pattern of the lymphoma.

Implications:

  • This case underscores the importance of considering PPL in the differential diagnosis of pancreatic masses, especially when imaging findings are non-specific.
  • Recognizing the rare diffuse infiltrative pattern of PPL is crucial for avoiding misdiagnosis and ensuring timely, appropriate therapeutic intervention.
