Chronic care delivery involves complex clinical events and significant medical expenses.
Existing methods often lack integrated views of clinical pathways and associated costs.
Empowering patients and providers with actionable information is crucial for navigating healthcare.

Purpose of the Study:

To develop a systematic methodology for embedding medical costs into clinical pathways.
To visualize the interplay between clinical events and costs in chronic care.
To provide actionable insights for patients and care providers.

Main Methods:

Analyzed electronic health records (EHRs) and medication costs for 288 patients (2009-2011).
Developed chronological clinical pathways (CPs) integrating care and costs using machine learning.
Categorized patients into subgroups based on clinical complexity and medication costs.

Main Results:

Identified 3 clinically pathway-based (CP-based) and 4 cost-based patient subgroups.
CP-based subgroups showed significant cost variations despite clinical similarities.
Cost-based subgroups revealed substantial differences in clinical needs.

Conclusions:

Clinical pathways enhanced with cost data offer detailed visibility into chronic care delivery.
This approach highlights variability and inconsistencies in current best practices.
