Pulmonary vein stenosis (PVST) after second-generation cryoballoon ablation is uncommon. This study found a low incidence of PVST, with no progression during follow-up, suggesting the technique is safe.

- Minimal and mild stenosis were observed in 6 and 4 PVs, respectively; no moderate or severe stenosis occurred.
- Stenosis predominantly affected the left superior PV (LSPV); no stenosis was seen in PVs with active balloon deflations.

- The incidence of PVST following second-generation cryoballoon ablation is low.
- PVST risk varies among different PVs, with LSPV being more susceptible.
- Specific procedural parameters like achieving -60°C or active balloon deflations were not associated with PVST.
