Hair growth begins with the production of keratinocytes by the basal cells of the hair bulb. Summary

Background:

  • Bicoronal incisions are utilized in various neurosurgical and craniofacial procedures.
  • Postoperative alopecia is a known complication following bicoronal incision techniques.
  • Limited research exists comparing alopecia outcomes across different bicoronal incision methods, especially concerning Raney clip usage.

Purpose of the Study:

  • To compare the incidence of postoperative alopecia associated with different bicoronal incision techniques: monopolar cautery, Colorado microdissection tip cautery, and cold steel.
  • To evaluate the impact of Raney clip use on the development of postoperative alopecia in patients undergoing bicoronal incisions.

Main Methods:

  • A retrospective study analyzed 505 patients who underwent bicoronal incisions between 1997 and 2015.
  • Exclusion criteria included pre-existing baldness and less than one year of follow-up.
  • Maximum alopecia width was measured and compared among techniques (cold steel, monopolar cautery, Colorado tip) and with/without Raney clip use.

Main Results:

  • Cold steel techniques resulted in significantly less alopecia (2.8-3.5 mm width) compared to monopolar cautery (3.8-4.3 mm) and Colorado tip cautery (4.9-5.9 mm).
  • The duration of Raney clip use was directly correlated with increased alopecia width (P < .001).
  • Colorado tip microdissection cautery was associated with the widest alopecia measurements.

Conclusions:

  • Preferential use of cold steel scalpels for skin and subcutaneous incisions can minimize postoperative alopecia.
  • Minimizing or limiting the duration of Raney clip use is recommended to reduce alopecia following bicoronal incisions.
