Summary

Electrocautery is a safe and feasible method for intersegmental plane dissection during complete thoracoscopic pulmonary segmentectomy. While operative time is longer, it shows comparable safety and efficacy to staplers for early lung cancer treatment.

Background:

  • Complete thoracoscopic segmentectomy is increasingly used for early lung cancer detection.

Purpose of the Study:

  • To evaluate and compare the safety and efficacy of electrocautery versus staplers for intersegmental plane dissection.
  • To assess outcomes in patients undergoing complete thoracoscopic pulmonary segmentectomy.

Main Methods:

  • Retrospective review of 58 patients undergoing segmentectomy (September 2013 - March 2016).
  • Patients divided into electrocautery (30) and stapler (28) groups for intersegmental plane dissection.
  • Comparison of preoperative, intraoperative, and postoperative variables.

Main Results:

  • Electrocautery group had a longer operative time (248.70±54.46 min) compared to the stapler group (209.39±67.25 min; P=0.017).
  • No significant differences were observed in intraoperative blood loss, total drainage volume, or chest tube duration.
  • Postoperative hospital stay and minor complication rates were similar between groups.

Conclusions:

  • Electrocautery is a safe and feasible option for intersegmental plane dissection in thoracoscopic segmentectomy.
  • Despite longer operative times, electrocautery demonstrates comparable safety and efficacy to staplers.
  • This finding supports electrocautery as a viable technique in minimally invasive lung cancer surgery.
