Summary

Immunoglobulin (Ig)G4 expression is found in a minority of primary cutaneous marginal zone lymphoma (PCMZL) cases. This study found no significant clinical or immunophenotypic differences between IgG4-positive and IgG4-negative PCMZL.

Background:

  • Primary cutaneous marginal zone lymphoma (PCMZL) is the second most common B-cell lymphoma affecting the skin.
  • Previous research indicated a high prevalence of immunoglobulin (Ig)G4 expression in PCMZL with plasmacytic differentiation.

Purpose of the Study:

  • To determine the incidence of IgG4 expression in PCMZL.
  • To correlate IgG4 expression with clinical and immunophenotypic characteristics of PCMZL patients.

Main Methods:

  • A multicenter study involving 30 PCMZL patients.
  • Immunohistochemistry and in-situ hybridization were used to assess IgG4, Ig light/heavy chains, and T/B cell ratios.
  • Findings were correlated with clinical features.

Main Results:

  • IgG4 expression was detected in 13% (4/30) of PCMZL patients.
  • IgG4-positive cases had a mean age of 69 at biopsy, with solitary lesions often on the trunk.
  • No significant differences were found in Ig restriction, B/T cell predominance, or lesion site between IgG4-positive and negative groups.

Conclusions:

  • IgG4 expression is present in a minority of PCMZL cases.
  • No significant clinical or immunophenotypic distinctions were identified between IgG4-positive and IgG4-negative PCMZL.
