Background:

Medullomyoblastoma is an exceedingly rare pediatric central nervous system (CNS) tumor, with limited documented cases.
Understanding the histopathological and ultrastructural characteristics is crucial for diagnosis and management.

Observation:

A unique case of medullomyoblastoma originating in the cerebellar vermis is presented.
Postmortem examination revealed extensive leptomeningeal dissemination and supratentorial metastasis.

Findings:

Histological examination, immunohistochemistry, and ultrastructural analysis confirmed skeletal muscle differentiation.
The presence of thick and thin myofilaments, Z bands (electron microscopy), and myoglobin (immunostaining) validated the myoblastic component.
No evidence of neuronal or astrocytic differentiation was observed.

Implications:

This case expands the understanding of medullomyoblastoma's rare presentation and aggressive metastatic potential.
Further research into the origins and behavior of these tumors is warranted for improved therapeutic strategies.
