A rare dedifferentiated liposarcoma in the retroperitoneum caused a duodenal perforation in an elderly male. This autopsy case highlights the potential for silent, advanced retroperitoneal tumors leading to fatal complications.

Background:

Liposarcoma is a rare soft tissue tumor, with retroperitoneal subtypes often diagnosed late due to asymptomatic growth.
Dedifferentiated liposarcoma (DDLS) is an aggressive subtype with a poor prognosis, particularly when presenting with advanced disease.

Purpose of the Study:

To report a rare autopsy case of a large, dedifferentiated retroperitoneal liposarcoma.
To highlight the association of this tumor with a fatal duodenal perforation.

Main Methods:

Autopsy examination of an 81-year-old male found deceased.
Detailed external and internal examination, including gross pathology and histopathological analysis.

Main Results:

Discovery of a large (48x30x6 cm) dedifferentiated retroperitoneal liposarcoma.
Identification of a 1.5x1 cm duodenal perforation with associated peritonitis.

Conclusions:

Dedifferentiated retroperitoneal liposarcoma can present asymptomatically and lead to catastrophic complications like duodenal perforation.
Autopsy findings underscore the importance of considering rare tumors in unexplained deaths, even in the absence of prior symptoms.
