Background:

  • Hepatitis A virus (HAV) infections are typically associated with liver disease.

Observation:

  • A 59-year-old man presented with balance disorder, lower limb paresthesias, and sphincter dysfunction.
  • Spinal MRI indicated acute myelitis.
  • Serological tests revealed Hepatitis A virus-specific IgM as the only significant finding.

Findings:

  • The patient's neurological symptoms were attributed to acute myelitis secondary to Hepatitis A virus infection.
  • High-dose corticosteroid treatment resulted in only partial neurological recovery.

Implications:

  • A systematic etiological investigation for Hepatitis A virus infection is recommended for all patients diagnosed with acute myelitis.
  • This is particularly important in endemic regions lacking widespread vaccine prophylaxis for HAV.
  • Early diagnosis and consideration of HAV in myelitis cases may improve patient outcomes.
