Intensive care unit (ICU) patients experience low light levels with a diurnal pattern, potentially disrupting circadian rhythms. Nighttime light exposure did not affect melatonin secretion in this study.

Purpose of the Study:

  • To characterize ambient light exposure in the ICU.
  • To identify light exposure patterns relevant to circadian regulation.
  • To investigate the relationship between light exposure and melatonin secretion.

Main Methods:

  • Light monitors recorded illuminance continuously for at least 24 hours per subject.
  • Plasma melatonin levels were measured hourly.
  • Vital signs and neurologic assessments were performed hourly.

Main Results:

  • 17 subjects provided 67,324 light data epochs.
  • Light intensity peaked in the late morning (median 64.1 lux).
  • No correlation was found between melatonin amplitude and light exposure.

Conclusions:

  • ICU patients have consistently low, diurnal light exposure.
  • Nighttime light exposure did not impact melatonin secretion.
  • Inadequate daytime light in the ICU may disrupt circadian rhythms.
