A rare CD5- and CD10-positive primary adrenal diffuse large B-cell lymphoma (PA-DLBCL) presented aggressive symptoms. Despite treatment, prognosis remains poor for this aggressive cancer subtype.

- A 69-year-old male presented with back pain, bilateral adrenal masses, and rib fracture.
- CT imaging revealed large adrenal masses (right: 6.5×3.6×7.0 cm; left: 8.1×4.8×6.9 cm).
- Histopathological diagnosis confirmed CD5- and CD10-positive PA-DLBCL with rib involvement.
Findings:
- The patient received EPOCH therapy with rituximab and intrathecal treatment.
- Tumor burden decreased significantly following chemotherapy.
- Disease progression led to death 8 months post-diagnosis.
Implications:
- CD5- and CD10-positive PA-DLBCL may indicate a particularly aggressive disease course.
- The prognosis for this specific subtype of lymphoma appears poor, even with current standard-of-care chemotherapy regimens.
- Further research into novel therapeutic strategies is warranted for aggressive PA-DLBCL subtypes.
