Area of Science:

  • Neurosurgery
  • Medical Education
  • Virtual Reality Applications

Background:

  • Surgical training relies on cadaveric dissection and simulation.
  • Virtual reality (VR) offers a novel approach to surgical rehearsal.
  • Anatomy-specific VR platforms can enhance surgical training fidelity.

Purpose of the Study:

  • To assess the impact of anatomy-specific VR surgical rehearsal on surgeon confidence.
  • To evaluate the effect of VR rehearsal on temporal bone dissection performance.
  • To correlate confidence levels with objective surgical outcomes.

Main Methods:

  • Prospective pre- and poststudy design involving 16 otolaryngology residents.
  • Assessment of surgeon confidence using pre- and post-VR rehearsal surveys.
  • Creation of anatomy-specific VR models from CT scans for virtual mastoidectomy rehearsal.
  • Objective grading of cadaver temporal bone dissections using a modified Welling scale.

Main Results:

  • 87.5% of residents reported a significant increase in confidence after VR rehearsal.
  • Confidence improvement was linked to perceived utility and varied with trainee experience and task difficulty.
  • Post-rehearsal confidence strongly correlated with graded dissection performance.
  • Residents rated VR rehearsal as having a moderate to high contribution to dissection performance.

Conclusions:

  • Anatomy-specific VR rehearsal enhances surgeon confidence in mastoid dissection.
  • The benefits of VR rehearsal are dependent on surgeon experience and task complexity.
  • Subjective confidence gains from VR rehearsal positively predict objective surgical performance.
