Area of Science:

  • Oncology
  • Plastic Surgery
  • Reconstructive Surgery

Background:

  • Breast conserving surgery (BCS) aims for cancer removal with clear margins and good cosmetic results.
  • Poor cosmetic outcomes are common in BCS for large cancers or limited breast volume.
  • Immediate volume replacement using lipofilling (autologous fat transfer) is an emerging technique.

Purpose of the Study:

  • To evaluate the cosmetic outcomes of BCS with immediate lipofilling.
  • To compare these outcomes with standard BCS in a pilot study.

Main Methods:

  • A pilot study involved 35 women undergoing BCS with immediate lipofilling.
  • Cosmetic outcomes were assessed using the Breast Q™ questionnaire.
  • Results were compared to a control group of 39 women who had BCS alone.

Main Results:

  • Patients in the lipofilling group had significantly larger cancers (median 21 mm vs. 16 mm).
  • At 36 months median follow-up, lipofilling group showed significantly better cosmetic outcomes (p < 0.001).
  • The lipofilling group also reported fewer local breast symptoms (p = 0.0045) with no local recurrences.

Conclusions:

  • BCS combined with immediate lipofilling yields superior cosmetic results compared to standard BCS.
  • This technique enhances patient satisfaction and reduces post-operative symptoms.
