The apparent diffusion coefficient (ADC) map and tumor contact length (LTC) can predict microscopic extracapsular extension in prostate cancer. Combining both measures offers improved diagnostic accuracy for staging.
Area of Science:
- Radiology
- Oncology
- Medical Imaging
Background:
- Accurate staging of prostate cancer is crucial for treatment planning.
- Microscopic extracapsular extension (ECE) is a key factor influencing treatment decisions.
- Multiparametric MRI (mpMRI) is increasingly used in prostate cancer assessment.
Purpose of the Study:
- To evaluate the diagnostic performance of the apparent diffusion coefficient (ADC) map and tumor contact length (LTC) in predicting microscopic ECE.
- To assess the combined diagnostic utility of ADC and LTC for microscopic ECE detection.
- To determine the accuracy of these quantitative MRI measures in patients undergoing radical prostatectomy.
Main Methods:
- Retrospective analysis of 92 moderate-to-high risk prostate cancer patients.
- Utilized receiver operating characteristic (ROC) curves to assess diagnostic performance.
- Evaluated ADC map values and LTC measurements as predictors of microscopic ECE.
Main Results:
- The ADC map showed an area under the ROC curve (AUC) of 72% (sensitivity 83%, specificity 61%).
- The LTC demonstrated an AUC of 74% (sensitivity 91%, specificity 57%).
- Combining ADC and LTC improved AUC to 77% (sensitivity 77%, specificity 61%). A logistic regression model achieved an AUC of 82%.
Conclusions:
- Quantitative measures like ADC and LTC enhance diagnostic accuracy in prostate cancer staging.
- Both ADC values and LTC are significant predictors of microscopic ECE.
- The combination of ADC and LTC provides the best diagnostic performance for predicting microscopic ECE.
