Summary

A 20-week home-based exercise program showed high adherence and effectiveness for improving quality of life and physical function in individuals with Tropical Spastic Paraparesis/Hamiltonian Myelopathy (TSP/HAM). The program was feasible and well-tolerated.

Background:

  • Tropical Spastic Paraparesis/Hamiltonian Myelopathy (TSP/HAM) is a neurological disorder impacting mobility and quality of life.
  • Limited research exists on the effectiveness of home-based exercise interventions for TSP/HAM patients.

Purpose of the Study:

  • To assess the feasibility and effectiveness of a 20-week home-based exercise program for individuals with TSP/HAM.
  • To evaluate the impact of the program on functional capacity, disability, and quality of life.

Main Methods:

  • Twenty-three TSP/HAM individuals participated, stratified by Timed Up and Go (TUG) scores (<20s vs ≥20s).
  • Participants engaged in a 20-week home-based exercise regimen.
  • Primary outcomes included exercise adherence, lower limb maximum voluntary isometric contraction (MVIC), Barthel Index, and SF-36 quality of life scores.

Main Results:

  • High adherence (90%) was observed for the home-based exercise program.
  • Significant improvements in MVIC and SF-36 social functioning were noted in the TUG <20s group.
  • The TUG ≥20s group showed significant improvements in SF-36 physical functioning.

Conclusions:

  • The home-based exercise program is a feasible and effective intervention for TSP/HAM individuals.
  • The program demonstrated positive effects on disability and quality of life in the studied population.
  • Mild to moderate adverse events were reported, but the program was generally well-tolerated.
