Background:

  • Sub-Tenon's anesthesia is commonly used for ophthalmic surgeries.
  • Its visual impact during procedures like pars plana vitrectomy (PPV) needs investigation.

Purpose of the Study:

  • To assess the visual effects of sub-Tenon's anesthesia during PPV.
  • To identify factors influencing these visual disturbances.

Main Methods:

  • Prospective case series of 104 patients undergoing PPV or combined phacoemulsification and PPV.
  • Patients reported light perception at various surgical stages.
  • Best corrected visual acuity (BCVA) and visual evoked potentials (VEPs) were measured pre- and post-operatively.

Main Results:

  • Most patients (95.2%) experienced temporary no light perception (NLP) during surgery, especially with contralateral eye coverage.
  • NLP incidence was influenced by eye coverage and photobleaching.
  • Postoperative BCVA and VEPs significantly improved compared to preoperative values.

Conclusions:

  • Temporary NLP is common during PPV with sub-Tenon's anesthesia, likely due to optic nerve conduction block.
  • These visual effects are transient and do not harm long-term visual function.
  • NLP incidence is not correlated with patient age, gender, or preoperative visual acuity.
