Background:

Malakoplakia is a rare, acquired condition characterized by the presence of Michaelis-Guttmann bodies.
Rectosigmoid involvement is uncommon, posing diagnostic and therapeutic challenges.
Optimal treatment strategies for rectosigmoid malakoplakia remain debated.

Observation:

A rare case of rectosigmoid malakoplakia was identified.
The patient presented with symptoms necessitating investigation of the rectosigmoid region.
Diagnostic workup confirmed the presence of malakoplakia in the rectosigmoid.

Findings:

The rectosigmoid malakoplakia was successfully treated with fluoroquinolone antibiotics.
Clinical and pathological resolution was achieved following the antibiotic course.
Fluoroquinolone demonstrated efficacy in managing this rare presentation of malakoplakia.

Implications:

This case suggests fluoroquinolone as a viable treatment option for rectosigmoid malakoplakia.
Further research may explore the role of fluoroquinolones in similar rare gastrointestinal inflammatory conditions.
Understanding successful treatment modalities is crucial for managing uncommon presentations of malakoplakia.
