Immunotherapy is a treatment that boosts or manipulates the immune system to fight diseases, including cancer. Summary

Rituximab effectively treats pemphigus vulgaris, offering prolonged remission and reducing the need for steroids. Early intervention with this biological drug shows greater efficacy in managing this autoimmune condition.

Background:

  • Pemphigus vulgaris (PV) historically had high mortality before corticosteroids.
  • Corticosteroids reduced mortality but caused significant morbidity due to side effects.
  • Novel molecular-targeted therapies offer improved efficacy and reduced side effects for PV.

Purpose of the Study:

  • To evaluate the effectiveness of Rituximab, a biological agent, in treating pemphigus vulgaris.
  • To identify and document adverse events associated with Rituximab therapy in PV patients.

Main Methods:

  • Open-label, prospective interventional study conducted in India.
  • Twenty PV patients (10 refractory, 10 new cases) received Rituximab.
  • Patients were followed for endpoints and adverse events post-administration.

Main Results:

  • Rituximab induced remission in 13 patients for 3-22 months.
  • Mean complete remission duration increased significantly with Rituximab (8.8 months) compared to conventional therapy (3.6 months).
  • Relapse occurred in 35% of patients, more common in those refractory to prior treatment.

Conclusions:

  • Rituximab is effective for pemphigus vulgaris, outperforming conventional treatments.
  • It reduces reliance on steroids and other immunosuppressants, inducing prolonged remission.
  • Early administration of Rituximab appears more beneficial; further research on sustained remission is warranted.
